CAPS Self Check-In Instructions

Checking In for Your Online or Site-Based Appointment

We want to make your check-in process for online and site-based appointments as smooth and easy as possible. Follow these instructions to get yourself checked in and start your visit. Call 520-621-3334 if you need any assistance.

Note for site-based counseling: To enable self-check-in, your appointment will appear as a Zoom visit (even for in-person visits). Check your secure messages in PatientLink for a message from your counselor confirming your appointment location.


Check-In Instructions

Before Your Visit

Before you begin, make sure you have:

  • A copy of your insurance card or a screenshot from your insurer's website
  • About 20 minutes to complete paperwork before your scheduled appointment time.

Step 1 

Log Into PatientLink at patientlink.health.arizona.edu.

Step 2

Update your INSURANCE CARD information.

If you have new insurance, have not given Campus Health your insurance yet, or aren't sure, you'll need to add your insurance card to PatientLink. You can update your insurance information in Patient Link at any time. 

Note about insurance information: Without the correct insurance information before your visit, you will be charged the out-of-pocket cost for your visit

Updating your insurance information:

  1. Click on the INSURANCE CARD tab in your Patient Link dashboard.
  2. Follow the instructions to upload the front and back of your insurance card (either a picture of the actual card or a screenshot of the card from your insurance carrier's website).
  3. You can add as many insurance cards as you like if you have more than one active insurance.

Step 3

Go to APPOINTMENTS and "check in" to your current appointment. Do not click JOIN yet - there is more paperwork to finish!

Step 4

Complete forms in the following sections of PatientLink: REQUIRED IMMUNIZATIONS AND FORMS, SURVEYS/QUESTIONNAIRES, CONSENT FORMS, and the QUESTIONNAIRE UNDER YOUR APPOINTMENT CHECK-IN INFORMATION.  Please note that these may not appear until you have checked in.

The forms required to start your appointment include: 

  • CAPS Screening Questionnaire (under your appointment check-in information)
  • CAPS Authorization to Bill (under Required Immunizations and Forms)
  • CAPS Consent to Treat (under Required Immunizations and Forms)
  • Text Messaging Consent (under Required Immunizations and Forms)
  • CAPS Telehealth Consent Form (under Required Immunizations and Forms)
  • CAPS Student Information Questionnaire  (under Surveys/Questionnaires)
  • CelestHealth BHM43 (under Surveys/Questionnaires; completed at every counseling or psychiatry appointment)
  • consent to see an intern form or associate licensed provider (not required for all providers, located under Consent Forms)

Step 5

Start your appointment.

Zoom: Go to APPOINTMENTS and click "join" to launch your Zoom meeting. You will enter a Zoom waiting room. Your provider will admit you from there. 

In-Person Site-Based Visits: please wait in the lobby for your counselor.
